Position Transition Checklist
- Proper transitioning of a team member's responsibilities to relief personnel who will serve in the same or higher level position during an incident is crucial. Personnel transition occurs as the SIMS organizational structure expands, contracts or as shift changes are executed. Transitions should be conducted face to face and done in such a manner to ensure continuity of incident response in order to maximize safety, situational awareness, and to ensure efficient management of the resources. Below is a list to assist you in your transition.

Required Items or Tasks Required Documentation or Forms Complete Ensure to properly check-in Form 211 Check-In List Determine your assigned SIMS Position Section Chief/Deputy, Unit Leader, Branch Director, Form 207 Organization Chart or IC Attain situational awareness Form 201-1 thru 5 Incident Briefing Forms, Operational Briefing, Section/Unit Briefing or Individual Briefing Verify incident name (IAP database login, if applicable) Planning Section Chief (PSC) Determine objectives and current progress Form 202 Incident Objectives Determine status of resources Form 201-4 Resource Summary Determine current and pending action items or assignments Form 233 Open Action Tracker, Form 214a Individual Log - team member you are relieving Determine Operational Period Discuss Situation Status Display with team member you are relieving Ensure Organizational Chart is up-to-date RESL, Unit Leader, Section Chief or Deputy Don your appropriate SIMS vest IC, Documentation Unit or Team member you are relieving Ensure the Communications Plan for accuracy Form 205 Communications Plan Announcement of transition of SIMS ICP announcement, assessment meeting or Shift Briefing Ensure that transitioning Command & General Staff have completed transition and check list if available Command & General Staff Briefing/Meeting or Shift Briefing
