Purpose: The Incident Action Plan (IAP) Cover Sheet documents the signature approval by Unified Command to implement the Incident Action Plan and may serve as a table of contents for the Incident Action Plan.

Purpose: The Incident Report provides the Incident Commander (and the Command & General Staff assuming command of the incident) with basic information regarding the incident situation and conditions from the field.

3. Weather Report

Purpose: The Weather Report form provides the Incident Commander (the Command & General Staff assuming command of the incident) with basic information regarding current incident specific weather conditions, forecast for the next twenty-four (24) and forty-eight (48) hour period.

Purpose: The Incident Map can be a map or trajectory showing the location of the incident & pertinent incident information depicting the situation.

5. Resource Summary

Purpose: Resource Summary listing of all resources dedicated to the incident which includes the name of supplier providing the resource, resource type (e.g. fire truck, boom, skimmer), description (e.g. size, name, capacity), quantity or amount of resource(s), area of operation (e.g. staging area, division, group, task force), status of each resource (e.g. Standby, En-route with Estimated time of arrival, At Staging, Assigned, & Out of Service).

Purpose: The Organization Assignment List provides IMS personnel with information on the units that are currently activated and the names of personnel staffing each position/unit.

Purpose: The Site Safety Plan is a site-specific document required by regulations. The Form 208 Site Safety Plan, at minimum addresses, includes, or contains the following elements: health and safety hazard analysis for each site task or operation, comprehensive operations work plan, personnel training requirements, PPE selection criteria, site-specific medical monitoring requirements, air monitoring plan, site control measures, confined space entry procedures (if needed), pre-entry briefings (tailgate meetings), pre-operations commencement health and safety briefings for all incident participants, and quality assurance of Form 208 Site Safety Plan effectiveness.

Purpose: The Incident Status Summary serves the following purposes: Used by Situation Unit personnel for posting information on Status Boards. When duplicated and provided to Command Staff members, it provides them with basic information for use in planning for the next operational period. It provides information to the Public Information Officer for preparation of media releases. It summarizes incident information for local and off-site coordination centers.

Purpose: The Decontamination Plan provides the procedures and resources needed to decontaminate personnel and response equipment.

Purpose: This form provides a general Safety Message to include in the Incident Action Plan.
18. Demobilization Plan Planning Demobilization Unit Operations & Unified Command Optional
Purpose: The Demobilization Plan provides the procedures and required approval process to release resources during an incident.
19. Form 230 Daily Meeting Schedule Planning Planning Section Chief & Situation Unit Leader Command & Operations N/A
Purpose: The Daily Meeting Schedule is use to record daily scheduled meeting activities, for information purposes.
20. Form 232 Resources at Risk Planning Environmental Unit Leader & Situation Unit Leader Scientific Support Coordinator RAR Optional
Purpose: The Resources At Risk Summary is used to record and identify details of the Situation Unit including Technical Specialist activity. It also serves as the Environmental Operations Plan which lists and prioritizes the protection of environmentally sensitive and socioeconomic areas during a response.
21. Traffic Plan Logistics Logistics Section Chief and/or Security Operations, Planning, & Safety Optional

Purpose: The Traffic Plan provides guidance for managing vehicle and/or vessel traffic during a response.
